The Diabetes Foundation annually honors individuals and organizations that have made significant and meaningful contributions to diabetes education and treatment in New Jersey.

The honorees receive an original work of art, commissioned by the Foundation, and based on the DFI logo. Selection of the winners is done by a nominating committee using the "Nobel Prize" model whereby each winner becomes part of the committee to select future winners. Each honoree is accorded a lifetime membership to the Diabetes Foundation.
